Application Process
The
following documentation must be provided by anyone
interested in receiving Low Income Weatherization
Assistance. Interested parties can either; call
their local service
provider or complete, print, and sign the necessary
forms contained on this website and send to their local
service provider.
The
following documentation is required to process your
application.
Completed Application
Signed Notice of Rights and Obligations
Copy of the three (3) most recent pay
stubs, or a copy of all benefit statements. (This
information must be provided for all persons residing in
the household.)
Copy of your power bill and/or gas bill (both
if applicable).
For homeowners, a copy of your deed or tax bill
showing ownership.
For renters, Weatherization Service Agreement
signed by your landlord.
Mail
your completed application packet containing the above
mentioned forms and documentation to the service
provider for your service area.
